F4R (2.0L 16v) Auxiliaries belt possible cause of breakdown?

The belt have gone of for the second time on our car. This time it ended up jammed under the now damaged timeingbelt. There no faulty pulleys and they are all aligned. Polishing shows the belt has been running on the outer groove's as they should.



The only other possible cause I have managed to come up with is that the belt have been traced around the water pump and driveshaft pulley in wrong order. There have been discussion about it at http://ift.tt/XMyAjV but i cant find much solid stugg about the issue anywere else..
